No, it can happen anywhere. This happened to me 20 years ago in Minnesota when I was in college. The ATM just off campus had been mistakenly stocked with $20 bills in the $10 slot. I took out $10 and it gave me $20. They corrected the withdrawal and took the right amount from my account after the fact.
