It's the other way 'round -- eating with a knife, as if
forks hadn't been invented, was backwoodsy and uncouth.
A taboo against putting your knife in your mouth persists to
this day; at one time it was "explained" that eating with a
knife would make people nervous that you might cut yourself!
More likely the taboo persists because people sometimes put
their knives into the common dish -- as if butter servers
had not yet been invented.
