2 other possibilities are that it may be either a hidden tape measure of some kind, or something to do with knitting (and that one I'm less sure of).
RMLG had the privilege of Loene McIntyre coming to one of our meetings back in 2004 with part of her antique sewing tool collection. She had all sorts of wonderful items, and one was a lovely carved or turned wood item that had a pull out form of tape measure inside. The tape was a wide -1-2"?- piece of linen/fabric that was marked out to a predetermined length - either 1" sections or whatever. The other tool that she had that was fascinating was a kind of shell looking carved wood piece that I think was to do with knitting - I think the yarn wrapped around it so that you could tuck it into your waistband? This tool is more ornate than the latter one above, but it's hard to see if there's a piece on the side that pulls out to draw out the measuring tape. Looks like a nice tool, whatever it is.
