this looks like a rights problem. Are you sure that your LAM admin
user has enough rights?

If yes then you should turn on logging to get more details. In
/etc/ldap/slapd.conf turn logging on with the line "loglevel 256".
OpenLDAP uses /var/log/syslog for log output.
