Your problem seems be in connect step, not in getting IP from the DHCP 
server, snoop/ethereal have no help. More helpful tool should be 
aircrack similar 802.11 frame sniffer...

Can you configure HostAP to monitor 802.11 frame exchange in association 
process? I recommand this debug method to you, although I haven't use 
HostAP before.

I don't think so. the issue is: when did this message happen? It must be 
after connection failure, right? if that's the case, this message should 
  not be the key.

Anyway, once I have a HostAP, I will investigate more.
