For gnome it shold be possible to edit the xml file: ~/.gnome2/vfolders/applications.vfolder-info

and then create ~/.gnome2/vfolders/applications/myapp.desktop files to include in applications.vfolder-info

it should be possible to add global entries also by editing /etc/gnome-vfs-2.0/vfolders/....

For kde take a look at ~/.kde/share/applnk it looks like it contains some more .desktop files that make entries in your kde menu
