Both. One must be very careful when choosing 
inks to avoid such artifacts. Even in 4->8 separations.
So, is common practice to sacrifice some of theoretical 
gamut extent in order to avoid that. Anyway I'm still 
learning about this subject :-)

do you really mean isolated holes _inside_ the gamut
solid, or just "dents" in the surface (i.e. only a
non-convex outer surface)?

I'm curious, have you actually seen such isolated
holes in practice for n-colorant (n > 4) profiles?
