Typically this is because the white point isn't quite correct. An accurate
colorimetric table will have the white point tag close enough to the device
white that the B2A returns zero CMYK. You should check whether the A2B
CMYK = 0 produces the white point first. If it doesn't, the white
point tags probably wrong. If it does, then possibly it's just the B2A
not matching the A2B with regard to the white point. This could
be due to poor accuracy of the B2A, or a poor choice of grid
locations (you have to arrange the per channel tables such that
the PCS white lands on an a & b grid line to get an accurate
white out of the B2A).
