We use http://www.ymyw.org/
It tracks Duty to God, Personal Progress, Faith in God, and Scouting (though 
the last is not apparent on the homepage).  The site is a bit slow, and I've 
just started using it, but it may satisfy these requirements (you would have 
to explore).  It uses role based login information so a leader can edit and 
track information, and a parent can see the progress of their children (and 
it appears that even each child can have a separate login).  See the "About 
this Site" link for important information on the site.  There is a "test 
drive" you can take on the site.
