I have had this due to forgetting to put the eth(x)  interface for the
DMZ into the /etc/Shorewall/MASQ file:

eth0      eth2  (dmz in my case)

Just a piece to check.  The bevaoir of every new system I put up fights
the dmz beyond what most reasonable men can stand.  I feel your pain
when it comes to getting the policy file to work as you think and then
put in the correct rule to allow an exception to the policy file.
Sometimes they feel upside down to me, or the policy file is broken.
