I think there may be an issue with the way some rich text/html email clients code for in-line attchments.
I certainly have an issue with receiving my email using Lotus Notes Web Mail and people sending me in-line file attachments. It appears that the sender doesn't encode the mime attachment correctly and/or the client program doesn't or can't extract the attachment. In my experience this only happens with in-line attachments, so perhaps try checking the way you attach the word file.
