*Eureka!!!* The difference is in a setting in Options>Customise>Sources, which 
gives a choice between linked and unlinked sources. Since the latter gives many 
more references, I never use it, and it is not the default, so I will now run 
my tests again.

  This is really strange eh? Why am I not getting the error everyone else is? 
It is definitely exporting the report into .rtf and it is definitely making 
footnotes at the end of each page on the .rtf. They are just goofy for AKA 
notations. Bugger!
