I have tested this and found that there is an intermittent problem when the total generations in the report creates multi-page reports. The first time you try to create such a report the Index may not be generated. If you close the report selection and then restart the report without making changes to your settings you may or may not get the index. All I can suggest as a work-around is to keep trying the report.
