I have restarted the report 6 times and have not been able to bring up the index (with just 9 generations). I guess I will wait until the bug gets fixed. Thanks for the info. I now know it is not me.

The first time you try to create such a report the Index may not be generated. If you close the report selection and then restart the report without making changes to your settings you may or may not get the index. All I can suggest as a work-around is to keep trying the report. I have submitted a problem report in our tracking system.
