You have to use a bit of lateral thinking here.  Search first for
everyone who *was* born in the US and tag the individuals on the
resulting list.  For argument's sake, let's say you use Tag 9.

Now search on 2 criteria, firstly that Indidivduals have a Death
Location containing "United States" AND that Individuals have Tag 9
equal to Untagged.

Tagging is a really useful adjunct to Searching and users with a
"problem search" should always look at the possibility of using tagging
to create or eliminate a sub-set.
