This is basically the same search as that suggested by Bruce, which btw. is the 
conventional way of looking for a negative. When I tested Chuck’s method, it 
did not work if the comma was included (see my second post)

See if something like this might get you started.

Qualifier.... this assumes that all individuals have a country of birth 
assigned. Other wise those with no birth info will be included in your final 
list.

First Search:
Individual   - Birth place  -  wildcard -   *, USA
Assign tag 1 to list
This list is those with USA birth locations

-----------

Second Search:
Individual    - Tag 1  =   untagged
Assign tag 2 to list
This list is the individuals with no indicated USA birth locations

---------------

Third Search:
Primary condition
Individual   -  tag 2  =   tagged

Secondary condition:
Individual    -  death place  -  Wildcard -    *, USA
Assign tag 3

This list should be those born outside the US, died in the US. May also include 
any individual where no born location is assigned.
