There isn't a "Memory" template. Just Personal Knowledge. and it can be anyone who relates their knowledge to you or even yourself.
It will say "Personal knowledge of...." with the name of the person. You can include their address in your source, but it will be suppressed in the printing. Give it a try to see how it looks. You can always back out of it or delete it.
