Yes they can appear to be out in the ocean, but in my experience this is usually when the maps are at low resolution. As one homes in to the location the placing becomes more accurate. There is one exception, however, when I first use the maps, and enter a British location which Bing does not recognise, then it puts me in the Atlantic Ocean west of Africa!
