You might try "The Handy Book for Genealogists" by Everton Publishing Company, Inc. (Logan, UT). My edition is a 1982 version but it gives a short history of each of the states and when each county was formed and which county it was from.

Specifically what was the name of the colony, when did it change, etc. Trying to go state by state is most difficult: just found that Connecticut was first named River Colony; also that it was formed from three colonies: New Haven, etc. Then there was the Dominion of New England.
