It means that they have been excluded from one
category or more of Potential Problems.

To see which, open the Individual edit form and
click the Exclude from Potential Problems button
- the asterisk on the button means that there is
a tick in one of the checkboxes on the following dialogue.

The double little arrows are turned on or off by Option 3.12

The person probably doesn't have any potential
problems shown by the little red circle because
they've been excluded. We could do this in Legacy
7. The new part of the Potential Problems in
Legacy 8 is that they show immediately with the
little red circle. Before you had to run a PP report.
