I think I know the problem. Those entries that failed to work had space, comma, space, comma, then the county name and state name. That meant that USA would have been one field too many, so it wasn't added. I am afraid if I use "find and replace" to replace the two space + commas, I will mess something else up. I should probably just go through one by one, to do it right.
