If you are referring to the SourceWriter templates, the drop-down lists describe the types of template which are available and you have to select what comes nearest to what you want. I'm not sure, as I don't know about the document you describe, but I'm guessing that Church Records>Church Record Books would suit your purpose. Bear in mind that you don't need to use *every* field on a template and you don't necessarily have to use a field for exactly what its fieldname suggests.
