Do you have lots of notes in your file. Notes can take up quite a bit of space since each note is allowed to be up to 1 MB (only used space is counted however so short notes will use little space and long notes will use a lot). Pictures will not add significant size because only the path to the file is in the database and that is only a few characters.

Have you done a file maintenance recently. File > File Maintenance > Check Repair will eliminate wasted space from deleted records and other changes you have made which you later removed. The database manager does not automatically recover space used by old records. I just did a check repair in my own file and reduced it by almost 1 MB from 19 MB to 18 MB with about 5,500 records.
