When I said you wouldn't change the source details every time you attach that source, by "source" I was meaning the particular record concerning the birth of John Reeves. If you find some other record in that same database, of course you change the source details. You're referring to another "item".

Yes, if citing a Death Register (or Index thereof) entry I would use
"entry for John Reeves' death, 1954" and attach the same
Source/Citation to all relevant fields - name, date of birth, date of
death, names of parents ... whatever is included in the database entry.
