I am experiencing EXACTLY the same problem. My symptoms: (1) preferred picture showing in family view (2) no pictures showing in media gallery (3) no pictures attached to this person according to picture gallery tool (4) in media gallery, no events have an * in the drop down indicating no pictures attached for this individual (5) In settings == Options > customize > 8.9 -checked box for Show the picture -radio button set All Preferred -check box set "If there is no picture at all, display a place holder silhouette"
I only seem to have this problem with one individual. I submitted a support request to look at this.
