I never found a real fix. My work-around was to attach the image in the picture gallery.
The Picture Gallery and Media Gallery really seem quite muddled at times.

I am experiencing EXACTLY the same problem.
My symptoms:
(1) preferred picture showing in family view
(2) no pictures showing in media gallery
(3) no pictures attached to this person according to picture gallery tool
(4) in media gallery, no events have an * in the drop down indicating no pictures attached for this individual
(5) In settings == Options > customize > 8.9
-checked box for Show the picture
-radio button set All Preferred
-check box set "If there is no picture at all, display a place holder silhouette"
I only seem to have this problem with one individual. I submitted a support request to look at this.

It sounds as if you simply attached to an event rather than direct to the individual. In the Media Gallery see a drop down arrow next to Individual. If you go up the list you'll find All which is All media linked to this individual (it doesn't include media linked to source citations for this person). This is a read only view.
If you look down the list, Events with media linked will have an asterisk.
Event media can show on Family View depending on your setting at Options - Customise 8.9

I have just added 32 photos to 32 individuals. Now for 31 of them clicking on the Media Gallery icon in an individual's toolbar displays the gallery as expected.
For the other one, nothing appears in the Media Gallery. But the picture is displayed alongside her name in Family View and in a Pedigree Report.
Checking in the Picture Centre I find no attached media for this one individual. Back to the family group and there she is!
