This is a bug introduced in the 12 April update that Dave says he has fixed now - so we wait for the next update or revert to the previous build if you have saved the full installation file.
Can't help with Item #1 as I don't use these Address options.

On the "Individual's Information" dialog screen you can click the "plus" sign at the end of the location and add a Birth Address and Birth Address Note. How do you get the Birth ADDRESS NOTE to print on Descendant Book Reports? (I am referring to the BIRTH ADDRESS NOTE, not the BIRTH NOTES which can also be added by clicking the plus sign). I am getting entries in my Location Index like the following. What does, for example, the "//179" mean? Why is the same location listed twice?
Martinez, Contra Costa County, California//179............................14 Milwaukee, Milwaukee County, Wisconsin....................5,6,7,11,12,14 Milwaukee, Milwaukee County, Wisconsin//8...........1,5,6,7,8,11,12,14
I've been away for a while and probably missed/forgot something. Sorry if I am repeating something. The Help File and a search didn't provide the answers.
