This has been a source (pardon the pun) of continuing problems for us, too.  We have 
had to adopt the somewhat cumbersome practice of adding an individual 'to-do' item 
describing each source modification so that after the merge operation the person in 
charge (that would be me) can go through the to-do list and double-check each and 
every source change to make sure we don't end up (as we used to) with three and four 
citations where only one is supposed to be.
