Until the Notes (general, medical, research) is expanded to more than a block of text and is allowed internal source notations, put it in an event -- Physical attributes or the like -- so it can be sourced, dated, etc.
"Event" is just a name; it can embrace any sort of record you want with careful fiddling of the sentence.
