[LegacyUG] PDF or Other Fillable Forms

I find that I can get people to co-operate to supply small bits of 
information if they can do it quickly and they will do it if the response 
can be automated.  To that end, is it possible to create a PDF file that 
can be converted to the fillable form type PDF to be returned to me?  Of 
course, I am talking about responding by email.

Or, is it possible to create a HTML document that would contain boxes to be 
completed and returned to the sender?
