Once you have created the PDF using Legacy you need to have a full copy of Acrobat ($$$). You can open your PDF in Acrobat, then add the fields/boxes yourself. It's tedious, but once you know how there are lots of shortcuts and techniques for doing overlays and duplicating fields. I use that technique for many things I do, but haven't tried it yet with a Legacy generated PDF.
