Publication Facts : Recorded on (date) with/by tape recorder You can add this into comments
Record was created in what (Town) or place
Repositary : Yourself-if you have (letter) or tape of interview or The Individual name
Comments: a brief explaination of interview/letter or whatever
This is a basic outline, arrange it the way it fits your sytle of sourcing. The way I have done it is:
Family Records: Oral History: (The name of the person)
