It is counting the matches in the minibuffer text widget not the body. The last test I did was to put 'hello hello' in the body of the currently selected node. I then used alt-x and typed how-many (enter) into the minibuffer. I then typed 'll' (enter) int the minibuffer. The report I got was that there was one occurrence of 'll' detected.
