The /tools/$LFS_TGT/include/c++/5.2.0 should have been installed by gcc.
Following error in config.log (in $LFS/sources/gcc-build) seems also
suggest the specified include-dir is not right.

That's your problem. /usr/include/stdio.h should be installed by glibc.
You could try to reinstall that. Remember to ensure the
glibc-build and glibc-2.22 directories are removed before re-extracting
the tarball.
Since you are so close to the beginning, I really suggest removing
/mnt/lfs/tools and recreating it:
echo $LFS
rm -rf $LFS/tools
mkdir -v $LFS/tools
chown -v lfs $LFS/tools
ls -l /tools
And then start Chapter 5 over.
