Well, it means the first of those, anyway.  ;-)

I need to think about the problem a bit more before I can fix it, and
it doesn't help that I'm down to the wire on some problems with an
application that we're currently only running on quad and hex meshes.

Do we have any diagrams of the tet and pyramid refinement patterns?
I don't recall them off the top of my head, whereas I do recall some
gotchas like the adaptive diagonal selection in tets and the
non-pyramid children of pyramid parents.
