Sadly, here's our first problem.  Configuring with --enable-everything
doesn't turn on --enable-parmesh for a reason: specifically, you don't
want to use ParallelMesh right now unless you're helping to debug
ParallelMesh.

Even more sadly: you're technically helping to debug ParallelMesh.  As
of yesterday, the only outstanding bug I knew of was an issue with
reconstructing mesh topology after an adaptive coarsening; it looks
like you've managed to trigger a failure with pure adaptive
refinement.

The workaround here is to switch back to SerialMesh.  I'm not sure
when I'll have time to work on ParallelMesh again, but unless I get
some serious volunteered help then "December at the soonest, next year
at the most likely" is a safe bet.
