This seems like a legitimate bug... it's like the System doesn't know about
the new element that you manually added. I tried
calling e->set_n_systems(1); but that just led to a different assert
elsewhere.
We usually only add new elements through MeshRefinement before calling
reinit(), but in that case they are flagged as JUST_REFINED and have a
valid parent Elem... I wonder if changing the "level 0" mesh in between
reinit() calls has just never been tested.
