A while ago, I searched for occurrences of "Major opcode of failed request" "(XVideo)". There are several relevant matches in help sections of forums and distro/project bug databases. People often attribute these XV errors to driver bugs. But the open NVidia driver doesn't have accelerated 3D support, and I don't have an Intel or ATI/AMD card on any other machine I own, so I gave up... The problem happens no matter SHM initialization is activated or not in the X11/XV/XV2 video output.
