Thanks for the suggestion, but neither Form Controls nor any other toolbar is
selected in LibreOffice Writer's View > Toolbars menu.

Even if it had been selected, it would still be wrong for several reasons:

*  With the LibreOffice Start Center bug duplication method, the Form controls
appear in Start Center, not in Writer.  Start Center doesn't have a View >
Toolbars menu item.  The Form Controls do not appear in the new Writer
document.

* With the Writer File > New bug duplication method, the Form controls briefly
appear and then vanish.  If the View > Toolbars > Form Controls was selected,
then one would expect it to remain on the screen and not immediately disappear.

* One might desire to have  View > Toolbars > Form Controls to remain on
between editing sessions in a given document.  However, it would be uncommon to
desire it to remain on in every new document.

I did some testing this morning.  If I turn on  View > Toolbars > Form
Controls, it automatically turns off when a document is closed.  That is I
created a new document, turned on  > Toolbars > Form Controls, added a Form to
the document, saved it, and then reopened it.  After reopening the document,
View > Toolbars > Form Controls was off. So on my Mac what you described isn't
occurring at all.  If this behavior is wrong, then it's another bug, or a
manifestation of the bug I originally described,
