That might be some issue with the specific keyboard layout (for which,
unfortunately, LO has poor support). If you have a numeric keypad in your
keyboard, try with that [+] key. An alternative for that issue would be to use
Calc > Tools > Customize > Keyboard; search for the "Insert Cells"  Function
for Calc (specifically, the function that corresponds to ".uno:InsCellsCtrl")
and assign to it [CTRL]+[+] and also [CTRL]+[SHIFT]+[+], in order to account
for your keyboard layout (whichever you have).

In addition to the keyboard shortcuts, you could always select multiple rows
before inserting the same number of rows that were selected.

Considering that there are fast-enough methods to achieve what was requested
(whether for the first row or on whichever other rows), adding one entry to the
Headers is not an adequate approach. The addition to the normal cell context
menu seems to be a compromise, I guess.

As I said already, the currently available methods take less time than it takes
me to write the comment, even if I were to perform the same kind of action
multiple times per day (which helps when learning frequent keyboard shortcuts
used for spreadsheet tools).
