All index entries are incorrect: they specify a blank key as a sub-category of a 1st key (which is in fact the intended key). Consequently, items are rightfully rendered on 2 lines: the sub-category (without page number) followed by the (void) key with its page number. Restructuring the index entries solves the problem (using the index feature as intended).
