Starting in LO 7.4, updating the table of contents ends up losing the second
Heading 2 entry. (I just noticed this while updating bug reports. It appears
that someone took this document to the chopping block, removing the Heading 1
as well as various other content. Now only two 'Heading 2' entries remain.)
Steps to reproduce:
-open 'LOBugNoteWidowOrphan (4).odt' (attachment 201767 from bug 167479)
-notice the TOC has a Heading 1 entry and two Heading 2 sub-entries
-update the TOC (right click - Update index)
Now the second 'Heading 2' on page 2 is no longer included.
