OK, thanks for clarifying. My observation now is:

- If the selected box is empty, the inserted image replaces the box (this is
definitely the right behavior). 
- If the selected box has some text, it's treated as a textbox, and the image
inserted as a background of it.

Any idea when it behaved differently? I tried with 4.4 and it's the same there.

I'm not sure. If there is a selected object, isn't the expectation that changes
will be applied to this object as much as possible, rather than creating new
floating objects?

Anyway, let's pass this to ux-advise.
