You state that the document was produced on OSX 10.7.5, then used in a MSWord environment, however, you do not mention whether this is MSWord on Mac, and which version that might be (2008 ? 2011 ?) or whether the document is opened in a Windows MSWord environment. You also do not say whether the document was initially created as an ODT, then exported to a MSWord format, or whether the MSWord environment you mention is attempting to import an ODT document (in which case, it is a Windows only setup, as Office for Mac doesn't support ODF). There is further the problem, that you are using an OS version which is no longer officially supported - 4.3.7 release is the final version for OSX 10.6-10.7, so any bugs (new or old) will almost certainly not get fixed.
