I'm not sure if this adds useful information or not.

Mid-2012 non-retina MacBook Pro with Dell U3014 Monitor.

Works fine without external monitor or if external monitor is scaled to 2048 x
1280 or less. Fails if monitor is run at native 2560 x 1600 resolution
