I didn't notice the bit about difference between different file formats in your report, so I tested with creating from a template, and both saving as ODT or DOCX, then exporting both as PDF. In both cases, the document title is kept from the template, so I can't reproduce your issue when comparing different file types. If you do not want this title to show, either remove it or update it in LibreOffice in File > Properties > Description > Title.
