Regarding (a), it is *not* possible to handle dot removal per voice, as my
example in #42 shows (attached is an image of it).
Regarding (b), to get it `right' it must be resolved as shown in the image
from comment #25 -- this is the *only* correct solution IMHO. However,
this should be handled in a different tracker issue, and it certainly is
beyond my (current) coding capabilities.
Please try the patch from #42.
