So here's the text from Gould, with her images. Note two key features: she says "When a dot is forced to be two or more stave-spaces from the chord... In such cases, use only as many dots as cover the number of stave-spaces taken up by the chord". Two or more. This is emphasised by the upper image, which shows dots one stave space away, and still left present. Note that she also reduces the number of dots to exactly match the chord height. If we accept her judgment, current master crops too soon; Werner's patch produces too many dots when cropping, and crops too soon; 2.16 doesn't crop and should do.

FWIW, Sibelius 7 does not follow the Gould recommendation. I attach an image of what it does (its PNG exports are horrible, too...)
