The code knows the extents and uses them for achieving the effect I
described. Basically, all columns are centered except for the left-most
and the right-most ones which are aligned on their borders.
I am looking through uses of \fill-line. It appears typical for creating
head/footlines with three columns at most. I would have leaned towards
letting spacing not count towards the number of columns. However, totally
empty fields are somewhat like spacing, and I am not sure that the typical
\on-the-fly header fields will not often turn out empty while not supposed
to affect overall layout. If that's indeed the case, perhaps spacing can't
be dealt with sensibly and has, if at all, to be placed inside the columns
itself.
