The docs example using NullVoice outside any staff, put it inside a
ChoirStaff, which does not accept NullVoice, so it did create a Staff by
default:

So probably best to apply choice (2) as well, making NullVoice act like a
Voice, and document by example how to use \accepts if you want a NullVoice
outside any enclosing Staff.
